Multipurpose Senior Services Program

Ever build a Lego set? All those parts connecting to create something wonderful.

That’s what Amir Erez does. As a social work care manager for Santa Clara County’s Multipurpose Senior Services Program (MSSP) site, Amir connects older adults with the resources they need to remain in their homes and communities. As an MSSP case manager with 20 years of experience, he says, "I conduct home visits, interact with seniors, and connect them with community resources."

Amir recently connected an MSSP participant with housing and meal service. He helped the county resident navigate the Santa Clara County Housing Authority’s process of qualifying for an apartment. He then arranged food delivery to ensure this individual’s nutritional needs were met and connected him with other support services to keep him safely in his new home.

"I derive great satisfaction from helping people who initially have limited access to services," Amir says. "Over time, I take pride in seeing them become informed about available resources and gaining the skills to navigate these services with minimal assistance. Case management, for me, is an ongoing process of adjusting plans to meet the evolving needs of each individual."

As case manager, Amir may refer participants to In-Home Supportive Services for state-funded care hours, VTA Access for transportation, and Adult Day Health Care Centers for socialization. He may also consult with a participant’s doctors for additional referrals, such as home health RNs, physical therapists, occupational therapists, or aides. It's all about those connections.

MSSP sites are another element of California’s 50-year tradition of empowering older adults.
