How To Find Services In My Area

As part of a grant from the Administration for Community Living, the California Department of Aging contracts with the Agency on Aging Area 4, the Sonoma County Area Agency on Aging, and the County of San Diego Aging & Independence Services to provide free No Wrong Door (NWD) navigation services to older adults and adults with disabilities.

You can locate the Area Agency on Aging (AAA) or the Aging and Disability Resource Connection (ADRC) in your area by selecting your county on the Find Services in My County page  of this website.

California’s Cal Community Connect (CCC) pilot project builds upon several successful system change initiatives driven by California’s Master Plan for Aging (MPA).

What Services Are Available

Cal Community Connect is a new program being offered in San Diego, Sonoma, Yuba, and Sutter Counties, designed to advance California’s NWD navigation system by providing older adults, adults with disabilities, and caregivers, streamlined access to Long-Term Services and Supports (LTSS) through a Community Health Worker (CHW) navigator.

This is a free program for older adults (60+) and adults with disabilities. A trained community health worker will provide comprehensive person-centered navigation services to assist older adults and adults with disabilities, locate and connect with community services, supports, and resources necessary to promote their health and wellbeing.

The community health worker will help with topics such as:

  • Health Navigation
  • Accessing community resources
  • Support enrolling in programs and services
  • Health Education
  • Disaster Preparedness

Program Term

June 1, 2025 – May 31, 2027

Who Can Get These Services

The Cal Community Connect program serves:

  • Older adults 60 years of age and older;
  • Adults with disabilities 18 years of age and older;
  • Caregiver(s) of an adult(s) 60 years of age and older;
  • Caregiver(s) of an adult(s) with disabilities 18 years of age and older.
  • Participants must be a resident of the state of California.

Individuals who meet these criteria are eligible to receive program services.

Language - Cal Community Connect community health workers are bilingual, speaking English and Spanish.

This program is supported by the Administration for Community Living (ACL), U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $450,000 with 100 percent funding by ACL/HHS.
