California Patient Representative Information System (CAPRIS) is a standardized data management system developed to initiate facility requests for Public Patient Representative (PPR) selection, upload resident information and required notices for PPR review, and collect data submitted by skilled nursing and intermediate care facilities. It is a secured web-based system that requires a two-factor authentication process to access. Only registered users can access the system with their email address and passphrase.
To register as a CAPRIS user, a facility staff person must receive prior approval from the facility administrator or his/her designee and complete the California Patient Representative User Action Request Form. Click on the document title below to access the form.
