Reporting Requirements

Whether Emerging or Designated, ADRCs file some reports with the ADRC Bureau annually. Other reports are filed only by Emerging ADRCs and others only by Designated ADRCs. Please see below for reporting requirements.

Emerging and Designated ADRCs: Reporting Requirements

ADRC Local Contacts

  • Frequency: Annually or as requested by ADRC Bureau
  • Due Dates: January 31 of each calendar year
  • Templates/Guidance:
    • CDA requires each local ADRC Core Partner to submit comprehensive contact information for individuals responsible for administration of local ADRC. See CDA 7066 and CDA 7066i forms that are downloadable below.
  • Submissions: Send to ADRC@aging.ca.gov

Annual ADRC Plan

  • Frequency: Annually
  • Due Dates: April 30 of each calendar year
  • Templates/Guidance:
    • CDA requires each local ADRC partnership to submit a plan detailing the goals, objectives, and planned metrics for the upcoming fiscal year. Please refer to CDA 7039 and CDA 7039i for forms and instructions.
  • Submissions: Send to ADRC@aging.ca.gov

Emerging ADRCs: Additional Reporting Requirements

Emerging ADRC Development Plan

  • Frequency: Quarterly
  • Due Dates:
    • July 31 of new fiscal year: new Emerging ADRCs submit initial development plan of goals for the fiscal year
    • October 31, for updated development plan indicating the progress made between July 1 - September 30
    • January 31, for updated development plan indicating the progress made between October 1 - December 31
    • April 30, for updated development plan indicating the progress made between January 1 - March 31
    • July 31, for updated development plan indicating the progress made between April 1 - June 30
  • Templates/Guidance:
    • CDA does not require a specific template for a development plan; however, you may refer to the sample template that is downloadable below for consideration.
  • Submissions: Send to finance@aging.ca.gov

Consumer Experience Survey (Optional for Emerging ADRCs)

  • Frequency: Report survey sample size of 50 or more consumers by July 31
  • Due Dates:
    • July 31, for surveys performed between July 1 and June 30.
  • Templates/Guidance:
    • The consumer experience survey is administered by local ADRC partners throughout the year.
    • Sample size of 50 or more consumers per year.
    • Please email ADRC@aging.ca.gov for survey questions available in English and 15 additional threshold languages.
  • Training:
  • Submissions:
    • Data is submitted to ADRC Bureau via Excel document. Please email ADRC@aging.ca.gov for a copy of the Excel document.

Designated ADRCs: Additional Reporting Requirements

Service Data Reports

  • Frequency: Quarterly
  • Due Dates:
    • October 31, for all activities that occurred between July 1 - September 30
    • January 31, for all activities that occurred between July 1 - December 31 (cumulative of first two quarters)
    • April 30, for all activities that occurred between July 1 - March 31 (cumulative of first three quarters)
    • July 31, for all activities that occurred between July 1 - June 30 (cumulative of all four quarters)
